Responsibilities

• Conduct comprehensive patient assessments to evaluate nutritional status, medical history, and dietary habits.

• Develop and implement individualized nutrition care plans aligned with medical diagnoses and treatment goals.

• Provide nutrition counseling and education to clients with diverse needs, including weight management, chronic disease management, geriatrics, and acute care cases.

• Collaborate effectively with multidisciplinary healthcare teams to integrate nutrition support into overall patient care strategies.

• Utilize electronic health records (EHR) systems to document assessments, care plans, progress notes, and follow-up actions accurately.

• Offer specialized nutrition support such as tube feeding management, dietary modifications for specific conditions, and medical nutrition therapy (MNT).

• Apply motivational interviewing techniques to encourage behavioral change and foster client engagement in their health journey.

Qualifications

• Bachelor’s degree or higher in Nutrition, Dietetics, or a related field; registered dietitian (RD) credential preferred.

• Proven experience working with individuals with specific dietary needs across clinical settings such as hospitals or long-term care facilities.

• Strong knowledge of physiology, clinical nutrition principles, and diagnostic evaluation methods.

• Hands-on experience with EMR/EHR systems for documentation and patient record management.

• Familiarity with nutrition support procedures including tube feeding and care plans for chronic diseases.

• Excellent client communication skills coupled with the ability to supervise or mentor junior staff members when needed.

• Demonstrated expertise in medical nutrition therapy (MNT), weight loss programs, and acute or geriatric care environments.
