Oakfield Surgery is a friendly, forward-thinking GP practice with a registered patient list of approximately 7,500 patients, serving the vibrant and diverse community of Penge within the London Borough of Bromley.

We are seeking a motivated, compassionate and enthusiastic Practice Nurse to join our supportive multidisciplinary team for 25 hours per week, preferably working Mondays, Wednesdays and Thursdays.

The successful candidate will provide a broad range of primary care nursing services, including chronic disease management, cervical screening, immunisations, health promotion, NHS Health Checks and wound care.

You will work alongside our existing clinical team, which includes a Senior Practice Nurse and Advanced Nurse Practitioner, as well as GPs, pharmacists, allied healthcare professionals and our wider practice team.

Main Duties of the Job

The Practice Nurse will deliver a range of clinical services within the practice, working within their professional scope of practice and in accordance with current national guidelines, evidence-based practice and local protocols.

The role will include:

Clinical Duties

Provide cervical screening in line with national screening programme guidance.
Administer adult and childhood immunisations and vaccinations.
Deliver wound care, dressings and treatment room services.
Undertake NHS Health Checks and other routine health screening.
Perform ECGs as required.
Administer long-acting injections, including Depo-Provera, Vitamin B12 and Zoladex, in accordance with appropriate care plans, training and practice protocols.
Support the management and monitoring of long-term conditions including diabetes, asthma, COPD and hypertension.
Support infection prevention and control procedures and participate in relevant audits.
Recognise and appropriately escalate any concerns regarding a patient's health or clinical condition.

Patient Care & Health Promotion

Provide health education and lifestyle advice to patients and carers.
Support patients with the self-management of long-term conditions.
Promote preventative healthcare, screening and immunisation uptake.
Encourage healthy lifestyle choices and appropriate use of primary care services.
Deliver compassionate, inclusive and patient-centred care.

Clinical Governance & Quality

Work within practice policies, protocols and national clinical guidelines.
Maintain accurate, contemporaneous and confidential clinical records using the practice clinical system.
Participate in clinical audits and quality improvement initiatives.
Contribute to significant event reviews and learning where appropriate.
Ensure the safe storage, handling and administration of vaccines and medicines in accordance with relevant procedures.
Maintain high standards of infection prevention and control.

Team Working & Communication

Work collaboratively with GPs, the nursing team, pharmacists, allied healthcare professionals and administrative colleagues.
Communicate effectively and professionally with patients, relatives, carers and external healthcare professionals.
Contribute positively to the wider practice team and support effective multidisciplinary working.
Participate in practice meetings and relevant clinical discussions as required.

Professional Development

Maintain professional registration and comply with NMC requirements.
Participate in supervision, training, appraisal and continuing professional development.
Maintain up-to-date knowledge and clinical competence within the scope of the role.
Take an active role in developing skills relevant to primary care nursing.
Undertake additional training where appropriate and supported by the practice.

General Responsibilities

Maintain strict confidentiality and comply with information governance and data protection requirements.
Follow practice policies and procedures, including safeguarding, health and safety and infection control.
Maintain professional standards of conduct at all times.
Work within the limits of professional competence and seek appropriate advice when required.

Person SpecificationEssential

Registered Nurse with current NMC registration.
Experience working as a Practice Nurse or in a relevant primary care/community nursing setting.
Good clinical assessment and decision-making skills.
Competent in core primary care nursing procedures.
Excellent communication and interpersonal skills.
Ability to work effectively as part of a multidisciplinary team.
Ability to work independently and manage workload effectively.
Commitment to delivering high-quality, patient-centred care.
Good IT and clinical record-keeping skills.
Understanding of confidentiality, safeguarding, infection prevention and clinical governance.
Commitment to continuing professional development.

Desirable

Experience in general practice.
Experience of chronic disease management, particularly diabetes, asthma, COPD and hypertension.
