As an experienced SCPHN you’ll use your specialist public health knowledge to lead the delivery of the Healthy Child Programme, build trusted relationships with families and work collaboratively with multidisciplinary partners to improve outcomes and reduce health inequalities. You’ll have the autonomy to make clinical decisions while being supported by a highly skilled and experienced team committed to delivering outstanding care.

Part of the main duties involves:

To organise the delivery of 0-19 service to a defined population, including home visits, school visits and provision at other sites

Undertake health needs assessments with children, young people and families and working in partnership with other agencies/services, to implement appropriate individual programmes of care

Undertake family centred enhanced support with families with complex health and social needs to improve outcomes for children, young people and families

As the only NHS Trust in London with women’s and specialist children’s services on one site we have unique opportunities to provide excellent, high-quality, integrated care for mothers, babies and children. We operate through four clinical directorates: Neonatology and Medical Specialties, Surgical Services, Theatres and Paediatric Intensive Care, Children’s Community Services and Women’s Services.

The post holder will have a leadership role within the 0-19 Universal Service and be responsible for managing a caseload, ensuring the provision of a high standard of service delivery to the school-age population and their families in the community.

This will include:

First contact, assessment, care and referral, identifying health needs and raising awareness of health and wellbeing.

Continuing care, health improvement programmes, chronic disease management and delivering the Healthy Child Programme.

Influencing the broader context which affects health and social wellbeing, providing public health and promotion programs that improve health and reduce inequalities.

The post holder will support the School Nurse Team Leader in developing effective relationships and maintaining effective partnership working and relationships’

The post holder will be responsible for education, training, mentoring, for health and social care students in clinical placements/he will mentor and be a preceptor to new junior members of staff.
