Join Our Team!

We are seeking a compassionate, organized, and resident-focused Social Services Director to join our long-term care team. The Social Services Director plays an essential role in advocating for residents, supporting their emotional and psychosocial well-being, and working closely with residents, families, and the interdisciplinary care team.

Position Responsibilities

  • Provide social services and psychosocial support to residents and their families.

  • Complete social services assessments, care plans, progress notes, and required documentation.

  • Participate in care conferences and interdisciplinary team meetings.

  • Assist residents and families with adjustment to the long-term care environment.

  • Advocate for resident rights, preferences, dignity, and quality of life.

  • Respond to resident and family concerns and assist with problem resolution.

  • Coordinate discharge planning and assist residents with transition planning and community resources.

  • Provide support during significant life changes, including illness, loss, and changes in level of care.

  • Maintain communication with families, representatives, community agencies, and healthcare providers.

  • Assist with advance directives, guardianship concerns, and other psychosocial needs as appropriate.

  • Ensure social services documentation is accurate, timely, and compliant with applicable regulations.

  • Collaborate with nursing, activities, dietary, therapy, and other departments to provide comprehensive resident-centered care.

  • Maintain confidentiality and protect resident privacy at all times.

Qualifications

  • Bachelor's degree in Social Work, Human Services, or a related field preferred.

  • Experience working with older adults, long-term care residents, or a healthcare population preferred.

  • Knowledge of long-term care regulations and resident rights.

  • Strong communication, organization, and problem-solving skills.

  • Ability to work effectively with residents, families, staff, and interdisciplinary team members.

  • Compassionate, professional, and resident-centered approach.

  • Ability to maintain accurate documentation and manage multiple priorities.

Pay: $62,000.00 - $67,000.00 per year
