We are seeking a compassionate, organized, and resident-focused Social Services Director to join our long-term care team. The Social Services Director plays an essential role in advocating for residents, supporting their emotional and psychosocial well-being, and working closely with residents, families, and the interdisciplinary care team.
Position Responsibilities

Provide social services and psychosocial support to residents and their families.

Complete social services assessments, care plans, progress notes, and required documentation.

Participate in care conferences and interdisciplinary team meetings.

Assist residents and families with adjustment to the long-term care environment.

Advocate for resident rights, preferences, dignity, and quality of life.

Respond to resident and family concerns and assist with problem resolution.

Coordinate discharge planning and assist residents with transition planning and community resources.

Provide support during significant life changes, including illness, loss, and changes in level of care.

Maintain communication with families, representatives, community agencies, and healthcare providers.

Assist with advance directives, guardianship concerns, and other psychosocial needs as appropriate.

Ensure social services documentation is accurate, timely, and compliant with applicable regulations.

Collaborate with nursing, activities, dietary, therapy, and other departments to provide comprehensive resident-centered care.

Maintain confidentiality and protect resident privacy at all times.
Qualifications

Bachelor's degree in Social Work, Human Services, or a related field preferred.

Experience working with older adults, long-term care residents, or a healthcare population preferred.

Knowledge of long-term care regulations and resident rights.

Strong communication, organization, and problem-solving skills.

Ability to work effectively with residents, families, staff, and interdisciplinary team members.

Pay: $62,000.00 - $67,000.00 per year
